Management of Biloma due to Biliary Tract Injury after Laparoscopic Cholecystectomy in a patient with Atypical Anatomy: A Case Report
نویسندگان
چکیده مقاله:
Introduction: Biliary tract injury is a major complication of laparoscopic cholecystectomywhich can be very challenging. But, the important part is to know the accurate anatomy of the ducts and site of the injury before any surgical attempt for ligation or repair. In this case a disaster might happen if we do not consider biliary anatomical abnormality as the main issue. Case Report In this case, there was an additional duct originating from the right hepatic lobe and inserting directly to the common bile duct (CBD) along with the right hepatic duct. The injury to this branch led to biloma secretion from two tearing sites, one proximal to the branch and another distal of the branch adjacent to CBD, during laparoscopic cholecystectomy. Conclusion There are numerous anatomic variations in the biliary tract, so according to this case we highly recommend to assay the exact anatomy before any intervention by means of Magnetic resonance cholangiopancreatography (MRCP) or intraoperative cholangiography.

BACKGROUND Jaundice presenting after cholecystectomy may be the initial manifestation of a serious surgical misadventure and requires rigorous diagnostic pursuit and therapeutic intervention. Biloma is a well recognized postcholecystectomy complication that often accompanies biliary ductal injury. متن کاملA rare case of hepatic subcapsular biloma after laparoscopic Cholecystectomy and subsequent endoscopic retrograde cholangiopancreatography
Background: Biloma is a rare abnormal localized accumulation of bile out of biliary tree due to an injury that occurs usually postoperatively from an injured cystic or bile duct. While most bilomas collect in the subhepatic space, we describe a rare case of hepatic subcapsular biloma after laparoscopic cholecystectomy and ERCP which was done one week after surgery successfully treated by p...

Methods-Results: Over the past two-year period the same surgeon performed 180 laparoscopic cholecystectomies without any intra-operative event. All patients but three had an uneventful postoperative course (morbidity 1.6%). In 3 cases (women, 72-74-88 years-old) the US-scanning revealed a sub-hepatic biloma manifested by persistent right upper quadrant or epigastric pain, abdominal distention, ...

INTRODUCTION Bile duct injury is a severe and potentially life-threatening complication of laparoscopic cholecystectomy. Several series have described a 0.5% to 0.6% incidence of bile duct injury during laparoscopic cholecystectomy. The aim of this study was to analyze the presentation, characteristics, related investigation, and treatment results of major bile duct injuries after laparoscopic ...
